CME Credits - My Account

Instructions for viewing and printing your CME transcript

To login and view or print data about your participation in CME activities, Click Here .

To login:

1. Type your last name in the User Name field.
   
2. Type the last 4 digits of your Employee ID# (a.k.a. M#) in the Password field.
   
   Note: If you do not know you Employee ID#, contact the Human Resources Department at (615) 327-6336.
   
   
3. Click the Login button. This displays your CME credit list.

Optional

If you would like to view a list of CME credits during a specific period oftime, enter the beginning date in the From: field and the ending date in the To: field. The dates hould be entered in the following order:

2 digit month - 2 digit day - 4 digit year
Example: For January 01, 2006 enter: 01/01/2006

Printing your transcript:

1. Click the Print Transcript link located in the lower left corner of the page.
